If you are a medical patient and you have submitted a medical recommendation, you authorize the use and disclosure of the medical information contained in the medical recommendation of your physician for medical cannabis for confirmation with the doctor by the center, from time to time. You also understand a copy of your record will be retained by Overland until the recommendation expires.  Overland’s policy is not to disclose the name or identity of any patient other that in the course of confirmation of the recommendation. You understand that you may have extra protection under California and Federal law with regards to your information. You expressly authorize the use and storage of this information according to the policy in this paragraph. You understand you may revoke this authorization in writing at any time and that the Overland will then only maintain a record as required by local and state regulations.
